Peter Attia's Exercise Protocol for Longevity: What He Does, Why He Does It, and What It Means for Your Training
Peter Attia has done more to bring rigorous longevity science into mainstream health conversation than almost anyone else working today. His framework for exercise — built around four pillars and anchored by the concept of the centenarian decathlon — is one of the most coherent and evidence-based approaches to physical longevity available. The four pillars framework
Attia organizes his approach to exercise around four interdependent pillars. The order matters — stability comes first not because it burns the most calories or builds the most muscle, but because it is the structural prerequisite for everything else.
Stability, in Attia's framework, refers to the ability to safely transmit force through the body — the neuromuscular control, spinal integrity, and joint stability that allows loading to be applied without injury. He recommends one hour of stability work per week, distributed as 5 to 10 minute blocks before each training session. His preferred methods include dynamic neuromuscular stabilization (DNS) and Pilates-based movement patterns. The argument is that most people who get injured in strength training do so because they attempt loading before they have adequate stability. Six months of focused stability work before adding significant weight is his clinical recommendation for most patients.
Strength is the second pillar. Attia recommends three 45 to 60 minute full-body resistance training sessions per week, targeting all major muscle groups with compound movements. His specific emphasis is on functional strength patterns — deadlifts, loaded carries, single-leg work, pulling patterns — that translate directly to the physical capacities required for his centenarian decathlon. He views strength as a retirement account: the reserves built in the 40s and 50s are what you draw on at 80.
Aerobic efficiency — zone 2 — is the third pillar. Four 45 to 60 minute sessions per week of low-to-moderate intensity cardio where conversation remains possible. The physiological target is mitochondrial adaptation: increasing mitochondrial density in skeletal muscle, improving fat oxidation, and building the aerobic base on which higher-intensity training operates. Attia has called zone 2 the single most important form of exercise he does for long-term metabolic health.
Anaerobic performance — zone 5 — is the fourth pillar. One 30-minute high-intensity session per week targeting VO2 max development. Attia has described VO2 max as perhaps the single most powerful predictor of longevity he can measure. The research supports this: the JAMA Network Open study of 122,007 adults found that elite cardiorespiratory fitness was associated with a reduction in mortality risk of 80 percent compared to the lowest fitness group. VO2 max develops through maximum-effort intervals, typically four-minute efforts at maximal sustainable pace separated by four-minute recovery periods.
The centenarian decathlon
The centenarian decathlon is the organizing concept that gives Attia's training its specificity and long-term direction. The idea: identify the ten to fifteen physical tasks you want to be able to perform at age 85 and train backward from those targets across the decades preceding them. Attia's personal list includes carrying 60-pound luggage overhead, getting off the floor from a lying position, hiking with 30 pounds of elevation gain, maintaining a 3-mile-per-hour walking pace on a treadmill for 30 minutes, and performing a single-leg squat to a chair.
These are not aspirational aesthetic goals. They are functional requirements for independence and engagement in the activities that define quality of life in later years. The centenarian decathlon framework demands that you train with specific functional targets in mind rather than exercising toward a generic notion of fitness.
What Attia's protocol emphasizes that most people miss
Three components of the Attia framework are consistently absent from conventional gym programming.
Stability first. Most people who start strength training add load before they have adequate neuromuscular control at the foundational movement patterns. The result is compensation, injury, and inefficient force transmission. Attia's insistence on stability as the prerequisite is backed by the clinical evidence from injury prevention research.
Power training. Attia explicitly trains for power — the ability to generate force quickly — through movements like jump squats, medicine ball throws, and high-velocity strength work. Power declines faster than strength with aging and is more directly predictive of fall prevention. Yet most conventional strength programs do not include dedicated power training.
Grip strength as a clinical marker. Attia tests grip strength in all his patients and uses the PURE study benchmarks as minimum standards. A two-minute dead hang for men at age 40, 90 seconds for women. A farmer's carry at 75 percent of body weight for one minute. These are functional tests that reveal the musculoskeletal health of the system, not just the specific muscles they test.
The measurement infrastructure that makes the protocol work
Attia's clinic measures VO2 max, grip strength, strength and stability assessments, mobility screening, and DEXA body composition. These measures together map a patient's current functional profile against their long-term trajectory and identify where intervention is most needed. This is the assessment infrastructure that makes his training protocol precise rather than generic.
Most people training under the Attia framework do the training without the measurement infrastructure. They track zone 2 heart rate through a wearable. They may have a DEXA scan. But they do not have a clinical benchmark for their functional strength-to-body-weight ratio, their power output relative to age-stratified norms, their single-leg balance performance, or their overall functional health score across the five dimensions that determine centenarian decathlon outcomes.
